ABSTRACT: Role of a 10-Gy boost in the conservation treatment
of early breast cancer: results of a randomized clinical trial
in Lyon, France
Purpose: To define the role of a 10-Gy boost to the primary tumor
in the conservative treatment of early infiltrating breast carcinoma
treated by limited surgery and radiotherapy.
Results: First results, with a median follow up time was 3.3 years
as of September 94 were published in JCO 1997. At 5 years, the
Kaplan Meier estimate of local relapse rate was 3.6% in the boost
group and 4.5% in the no boost group (p = 0.044). After adjustment
for the main prognostic variables such as presence of an extensive
in situ composant in the periphery of the tumor, maximal diameter
of the tumor, degree of histologic differentiation, age at the
inclusion, no free margins, the relative risk was still significantly
lower for the boost group.
In September 2000, Bartelink publish the first results of the
EORTC boost trial with comparable results, demonstrating that
a boost significantly improves local control.
Currently, we are updating our results in terms of local relapse
rate, disease free survival, metastasis free survival, and overall
survival.
The occurrence of local relapse was recorded, to day in 768 patients
at 10 years: 26 in the boost group suffered a local relapse compared
with 36 in the control group. However only 4 patients dead of
cancer in the control group versus 10 in the boost group.
Conclusion: Results of the whole group will be updated at the
time of the meeting.
