Coagulation behaviour at starting and at stopping adjuvant post-operative tamoxifen (T) in 38 subjects with breast cancer (BC)

Abstract: Rationale: T is largely used as an adjuvant therapy after complete removal of primary BC. T may negatively influence the coagulation behaviour. Vein thrombosys may occure, even if rarely.

Patients and tests: We have prospectly studied 38 patients treated with oral 20 mg/day T for 5 years: 37 women aged from 41 to 77 y.o. (meanąSD: 56.97ą9.50) and a male patient aged 58 y.o. Initial age: 54.38ą9.68; age at therapy end: 59.3ą8.95.

Tests (see table) (meanąSD values) were measured before starting (B1) and after stopping (B2) the T and at intermediate times. Also the pharmacokinets of T has been studied (data not shown). (table) Student paired t-test has been used: p<0.0001: fibrinogen, b vs a// p<0.001: fibrinogen, c vs a; ATIII, c vs a; C-protein, c vs b// p<0.01: fibrinogen, c vs b; C-protein, c vs a; S-protein, c vs b, c vs a, b vs a, e vs d; ATIII, e vs d// p<0.05: fibrinogen, e vs d, PTT, e vs d; C-protein, e vs d.

Conclusions: T induced a consistent modification of the coagulation in terms of fibrinogen consumption. It resolved after discontinuing therapy.

Caution in the use of primary preventive T therapy is mandatory.
